DETAILED GUIDANCE FOR FILLING UP THE FORMS (ANN.III AND FORM OF APPLICATION):

ANNEXURE-III
This form is required to be filled, signed and sent to New Delhi and DG/RDSO by the Bank in terms of Para 4 of the OM dated 14.10.2008 issued by the Dept. of Pension & Pensioners' Welfare, Min. of Personnel, Public Grievances &Pensions.
Sl. Nos. 1 to 7 can be filled from the details available in your PPO and details for Sl. Nos. 7 to 9 are your S.B. A/c where the pension is credited.
PART - A (Pension)
A – Here mention the amount of your pension as mentioned in the PPO
B – Dearness Pension is 50% of the pension amount
C – It is already mentioned, 24% of A + B above
D – This is 40% of A above
E – This is A + B + C + D above and this is your consolidated pension as applicable from 1.1.1996
PART – B (Pension)
Sl.No. 11 (i) This is 50% of the minimum of the pay scale + Corresponding Grade Pay as shown
in Annexure-I – Col.6 and 7.  This is already indicated in Col.8 of Annexure – I.
Sl.No.12 – Here mention the amount of Part-A (E) or Sl.No.11(i), whichever is higher.
PART – A (Family Pension)
A – Here mention the amount of family pension as mentioned in your PPO.  There will be two
heads one upto 7 years from the date of retirement and the other on completion of 7 years.   As on 1.1.1996 if you have not completed 7 years, you have to mention amounts under 2 heads  (i) upto the date prior to completion of 7 years and (ii) on completion of 7 years.
If you have completed 7 years of retirement before 1.1.1996, then you should show one heading only, i.e. the amount mentioned in the PPO on completion of 7 years.
Likewise the data against B, C, D & E under Family Pension shall be either in one heading or 2 heads, i.e. (i) before 7 years and (ii) after 7 years.
The Enhanced family pension is upto the date of completion of 7 years and the family Pension is from the date of completion of 7 years as mentioned in the PPO.
Likewise in Part-B, against Item 11(i) – this is 30% as in Part-A on completion of 7 years.   This is also indicated in Col. 9 of Annexure – I.  Here also the mention of data against 2 heads or one heading before and after completion of 7 years shall apply.
Sl.No.12 will be same against Part-A (E) or Sl.No.11(i), whichever is higher under 2 heads or one heading as the case may be.
Give this Annexure.III (minimum of 3 copies) duly filled in by you, to the Centralised  Pension Processing Centre (CPPC) of your Bank.  As mentioned in Para 4 of the O.M. dated 14.10.2008, the CPPC of your bank should send one copy each to Delhi Address and DG/RDSO and they should get acknowledgement from them.  You can request the Bank to give you one copy for your use.  After getting the Annexure-III duly signed by the CPPC of your Bank, attach a Xerox copy of the same along with the Form of Application to be sent to Establishment Section of RDSO/LKO.   Retain the original copy of Annexure-III obtained from the Bank for your records.

FORM OF APPLICATION
Sl.Nos. 1 to 9 can be filled from the details available with you.  Reg. Sl.No.10 you have to mention the addresses of the pension drawing bank along with Branch Code and the CPPC address.  Also attach a Xerox copy of the proof of date of birth of your wife duly attested as required against Sl.No.5.  Against Sl.No.11 mention the details of enclosures.  
Send all these under a covering letter to RDSO/LKO for further necessary action and follow it up periodically.
If you are not able to get the Annexure-III signed by your Bank, fill up the FORM OF APPLICATION, enclose Xerox copies of PPO and Proof of date of birth of spouse, sign and send them to DG/RDSO/LKO  by Regd.Post Ack. Due.  If you hand over the same to the Estt. Section personally, get an acknowledgement from them for your records.
